Before you begin to complete this form, you should know or have ready:
- your licence details
- survey information for each site
- the total number of hazel dormice that you or your accredited agents captured or disturbed at each survey site
- the peak count of hazel dormice that you or your accredited agents observed during a single survey at each site
- details of any welfare concerns, such as hazel dormice that were injured or killed
- information about any accredited agents who worked under your licence
