- survey information for each site
- the total number of hazel dormice captured or disturbed by each licensable method at each survey site
- the peak count of hazel dormice seen during a single survey at each site
- details of any welfare concerns, such as hazel dormice that were injured or killed
- information about any accredited agents who worked under your licence
