Before you begin to complete this form you should know or have ready:
- your licence details
- survey information from when you captured or disturbed bats
- the total number of each bat species captured or disturbed by each licensable method at each survey site
- the peak count of each bat species seen during a single survey at each site
- details of any welfare concerns such as killed or injured bats
- information about any accredited agents who worked under your licence
