Before you start

Before you begin to complete this report of action form, you should know or have some information ready. The information required depends on the type of report you're submitting.

For a 14-day return report, you should know:

  • your licence reference number
  • the fishery name, location, county, post code and minimum 6-figure Ordinance Survey grid references for each location where the licence has been used
  • the name of any assistants used
  • the details of otters in the fishery (date they were reported in the fishery, where they entered and the number)
  • details of the trapping carried out

For an annual report of action, you should know:

  • that a 14-day report for all sites you've used the licence has been submitted
  • your licence reference number
  • if you want to stay registered
  • if any of your contact details have changed
  • details of any wildlife convictions, if applicable
