Use this form to apply for a new A08 or A09 individual licence.

You may be asked to submit supporting evidence with your application, including:

• evidence of the problem caused by the wild birds - read the A08 and A09 guidance (opens in new tab) for examples of evidence you can provide
• a log of bird observations at the site, including records of any non-lethal actions (opens in new tab) you've used
• a site map showing where you plan to carry out bird control - instructions later in this form explain how to create your map
