Before you begin to complete this form you should know or have ready:

  • your licence details
  • the total number of white-clawed crayfish caught at each survey site
  • the numbers and types of non-native species caught at each survey site
  • details of any welfare concerns such as killed or injured white-clawed crayfish
  • information about any accredited agents who worked under your licence

White-clawed crayfish are also called Atlantic stream crayfish.
