Before you begin to complete this form, you should know or have ready:
- your licence details
- details of any welfare concerns, such as water voles that were injured or killed
- information about any accredited agents who worked under your licence
For each site where you took action, you will need to provide information about:
- the site location
- temporary and permanent impacts
- the dates when actions were taken
- destructive searches completed
- compensation and post-development monitoring
- mink presence and mink control programmes in the area
- other predators at the site
