This form allows you to apply to export live fish and shellfish, as required under the Aquatic Animal Health (England and Wales) Act 2009.
Completing this form is the first step in the process. The next step will be a site visit, which will normally occur within 7 days after we receive your application. An inspector from FHI will call the applicant directly to arrange a date and time for the site visit.
